                 VASER® for Hyperhidrosis – Excessive Sweating

There are new techniques such as the Vaser which can be used to treat axillary hyperhidrosis (Excessive sweating of the armpits). They are a minor surgical procedure carried out under local anesthetic where 1 small incision is made. The laser/vaser heats the tissue and  destroys the sweat glands. The results are semi-permanent and there have not been any reported complications to date.
 
Vaser Ultrasound treatment has been been found to produce a permanent reduction in sweat production with no downtime.  There’s no tissue damage and despite a slight soreness immediately after the treatment, everything is usually pretty much back to normal in a couple of days. You can reduce the sweating but not completely wipe it out and it is temporary.
 
How does the Vaser Sweat Therapy work?
 
First, the underarm area is filled with a special saline solution that helps numb the site and shrink the blood vessels (to minimize blood loss and reduce bruising). This solution wets and fills the area to be treated, making it easier to break up tissue with the VASER System’s ultrasonic energy. Then a small probe transmits sound energy (similar to that used for cataract removal from the eye) to break up fatty tissue on contact.  With the application of a specialized technique, the ultrasonic energy is concentrated on the sweat glands in the armpit while preserving other important tissues.  Finally, the liquefied fat and sweat glands are removed through a gentle suction process designed to minimize damage to surrounding tissues.

Below is a brief overview of accepted local surgical techniques for axillary (underarm) hyperhidrosis (excessive sweating):


Curettage: In small Taiwanese study of 19 patients (90%) had excellent to good response.


Liposuction: Sweat reduction rates range from 44% to 49%


Liposuction and Curettage: Sweat reduction rates approximately 63%


Laser: Reduction of sweating rated as "excellent" by >75% of patients studied, rated as "good" by 51-75% of patients, and rated as "fair" by 26-50% of patients.

Each of the above treatments has potential complications, which your physician should tell you about.

Sweat glands are not like tumors or lesions that we can see and remove easily. It's not going to work for everyone.

If nothing else works the patient might want to consider ETS.
Please note that in most cases Doctors do not recommend endoscopic thoracic sympathectomy (ETS) for excessive underarm sweating. 

While ETS is perhaps the most commonly discussed, (as well as criticized) surgery for hyperhidrosis, there are other, far less risky, and far more predictable, options for people who suffer from excessive sweating of the underarms.

As a last resort Dr. Gary Rothfeld refers patients to Harvard Medical School and Massachusetts General because it has one of the best centers for endoscopic thoracic sympathectomy (ETS) for excessive underarm sweating in the world.
