                     Follicular Unit Extraction (FUE)

Hair Transplant. Follicular unit extraction or FUE is a hair transplant technique in which a small round punch is used to extract follicular units from a patient's bald resistant donor areas. 
The cost per graft of FUE is also typically twice the cost of the standard follicular unit hair transplant procedure in which a strip of donor tissue is removed from the back of the head and trimmed under magnification into individual follicular unit grafts.
 During the Follicular Unit Extraction or FUE procedure individual follicular unit grafts are excised one at a time using a tiny one millimeter or less sized punch.

Since the direction and angle of the follicular unit beneath the skin can't be seen and can often differ from the direction of the hair on the surface, a sharp punch if used below the surface of the skin might transect or severe the underlying follicular unit.


Comparing Follicular Unit Extraction (FUE) to the Standard Strip Procedure.

While the FUE procedure has grown in popularity, largely due to the minimally invasive way in which follicular unit grafts are removed, the standard strip excision method is still the most popular hair transplant procedure. Donor strip being excised Incision sutured together Hair combed down after suturing The standard strip excision method involves surgically removing a strip of scalp from the bald resistant donor area of the patient. 

Small groups of hair follicles (mini-grafts) and individual hairs (micro-grafts) are cut from the back of the scalp and transplanted to recreate or enhance a natural looking hairline and provide hairs or increase density to the bald or balding areas of the scalp. Since the original location of the hair follicle determines its fate, the transplanted hairs grow as they would normally in their original location. 

Modern technique, the use of large numbers of very small grafts, and new medications that to a certain extent slow down the balding process, have made hair transplants a very useful and cosmetically sucessful procedure. 

While mega sessions (1000 - 2000 grafts per session) are appropriate for some patients, better results with less complications are often achieved with multiple (2 - 4) smaller sessions. During each of these sessions 400 to 700 grafts are done, with an interval of two to three weeks between sessions. Recovery time is four to five days. The average costs to restore a balding scalp can range from $10,000 to $20,000. As with any procedure, complications such as infection and scarring can affect the final cosmetic result.

In many cases, there are ways to treat both male and female hair loss. It all depends on the cause. Here are some common and not-so-common reasons why you might be seeing less hair on your head.

Physical stress
physical trauma—surgery, a car accident, or a severe illness, can cause temporary hair loss which can cause a telogen effluvium
 Hair loss often becomes noticeable three-to-six months after the trauma. Eventually the hair will grow back after the body recovers.

Your hair will grow back in a couple of months. 

Lack of protein

Male pattern baldness
classic pattern in which the hair recedes at the temples, leaving an M-shaped hairline.
topical creams like minoxidil (Rogaine) and oral medications such as finasteride (Propecia) that can halt hair loss or even cause some to grow 
surgery to transplant or graft hair is also an option
Female-pattern hair loss,
women don't tend to have a receding hairline, instead their part may widen and they may have noticeable thinning of hair
women may benefit from minoxidil (Rogaine) to help grow hair, or at least, maintain the hair you have,

Female hormones
pregnancy hormone changes can cause hair loss, so can switching or going off birth-control pills. This can also cause telogen effluvium, 

Emotional stress
Emotional stress is less likely to cause hair loss but will exacerbate a problem 

iron deficiency (the most common type of anemia), which is an easily fixable cause of hair loss

when it’s not pumping out enough hormones, can contribute to hair loss

Autoimmune-related hair loss

Steroid injections are the first line of treatment for alopecia areata, which appears as hair loss in round patches on the head.

Vigorous styling and hair treatments over the years can cause your hair to fall out. Examples of extreme styling include tight braids, hair weaves or corn rows as well as chemical relaxers to straighten your hair, hot-oil treatments or any kind of harsh chemical or high heat

Seborrheic Dermatitis
inflammatory scalp and skin condition . Severe and persistent seborrheic dermatitis can cause hair loss.

A significant percentage of my Hair Loss and Dermatology Conditions are diagnosed  on listening to medical history of the patient alone, a further percentage on examination and the remainder on further investigation and my patients teach me the rest. It is clear to me that history and examination skills remain at the very core of my dermatology practice.

Older Dermatologists like myself rely on history and physical to a greater degree than the younger dermatologists.  I think that the pendulum has swung too far towards all these cosmetic procedures where the younger dermatologists are just doing cosmetic procedures and not listening to their patients.  This is an important point because if the Dermatologist does not spend enough time with the patient the hair and cosmetic procedures will not work. In the case of a hair loss patient you must be almost 100 per cent certain why that patient is losing their hair if the patient is to have any chance of slowing down their hair loss or growing back their own hair.

It is vital to undergo a full pre assessment to ensure that your skin type is fully assessed and to be  accurate about your medical history. This particular procedure might not be the right treatment for you.  It is crucial that the Doctor does a full History and Physical Examination and listens to your concerns.  It very important to understand the individual needs of each patient because one procedure could work on one patient and then not another.
