Age spots (liver spots) (solar lentigo) typically develop as a result of prolonged exposure to ultraviolet light. They are usually the product of sun exposure, but tanning beds and other tanning equipment may also contribute to their development. By definition, age spots are not cancerous and present no physical health problems, but they can be a cosmetic concern. 

However, for cosmetic purposes, Dr. Gary is an expert in treating age spots. Age spots (also called liver spots or solar lentigo) are collections of pigment caused by exposure to the sun. Pigment is deposited as a response to injury, just like a scar is a response to a cut. The pigment collects in areas injured because of thin skin or greater sun exposure. 

Age spots also can be caused by bruising that leaves blood pigment behind. They are most common in people older than 55. The spots commonly appear on the hands, but they can be almost anywhere, especially sun-exposed areas, such as the face, back, arms, feet and shoulders. flat, brown areas are called lentigines. 

They have nothing to do with the liver - they are caused by the sun and usually appear on the face, hands, back and feet. They are generally harmless. They may look like melanoma and therefore may require evaluation.
 Commercial "fade" creams will not make lentigines disappear, but effective prescription medications and surgical resurfacing treatments are available.  We treat these age spots or brown spots with stunning results.

Sun-induced skin damage can cause brown age spots, especially on oft-exposed areas like the hands and face. At one time, the only remedy was to cover them up with cosmetics. 

Now, there are therapies that help reverse the signs of photoaging at the physiological level. One approach is physical removal by surgery, chemical peels, electrosurgery, gentle topical medications,lasers such as Q-switched Alexandrite laser, triple bleaching creams,Topical tretinoin creams which take several months, and many other therapies.

To get the best results, you should wear a sunscreen with an SPF of 30 or higher containing titanium dioxide and zinc oxide (Blue Lizard and Bull Frog) at 10 AM in the morning and 2 PM in the afternoon indoors and outdoors when the sun is the strongest. The sun penetrates through the windows in your home, work and your car.

Tretinoin. Topical tretinoin was first approved for treating acne, but trials have demonstrated that it also improves photoaged skin. It can take several months to lighten age spots, and side effects include redness, scaling, and itchiness, although these generally subside after a few weeks. 

Adapalene gel. This prescription drug is approved only for treating acne, but sometimes it is used off-label to improve photoaged skin.

Chemical peels, chemical spot treatments, laser treatments, electrosurgery, light therapy, and many other treatments have been used to lighten age spots. Some of these procedures require just one treatment; others take two or three sessions.

Age spots or brown spots usually first appear on the face and are the result of genetic susceptibility as well as sun exposure. 
Usually one to two treatments is adequated to remove the brown spots anywhere on the body. 

Freckles on the face, chest, and back can also be effectively treated with electrodessication.

Older Dermatologists like myself rely on history and physical to a greater degree than the younger dermatologists.  I think that the pendulum has swung too far towards all these cosmetic procedures where the younger dermatologists are just doing cosmetic procedures and not listening to their patients.  This is an important point because if the Dermatologist does not spend enough time with the patient during the consult the cosmetic procedures will probably not work.
